Comment and Feedback
If a beneficiary you enrolled in a Care Improvement Plus plan is contacting you with questions that should be answered by a Member Service Representative, please submit an e-Comment and Feedback Form.
To submit a form, complete the fields below and include the member's question/comment and his/her contact
information (i.e. first name, last name, Member ID number - if available, etc.) in the
Your Comment/Question box and click Submit Request. Or, instruct the member to call the Member Service
number on the back of his/her card, which is 1-800-204-1002 (TTY: 711) 7 days a week from 8:00am – 8:00pm.
