With every Care Improvement Plus plan you get a choice of plan options.
We offer a choice of options for the Care Improvement Plus Chronic Condition Special Needs Plan.
- Silver Rx (PPO SNP) - for those with diabetes and/or heart failure and who also have Medicare and Medicaid
- Gold Rx (PPO SNP) - for those with diabetes and/or heart failure and who do NOT receive Medicaid
Additional services - well beyond what's provided by Original Medicare:
Care Improvement Plus has an Open Access Provider Network, with no referral requirements for Medicare covered services. This lets beneficiaries go to any Medicare-approved provider that will accept payment from our plan. In addition, beneficiaries also get:

- 24/7 Nurse Hotline
- Preventive health care
- Emergency care
- Durable medical equipment benefits
- Social Service Coordinators program – to help members better manage out-of-pocket expenses for health care, prescription drugs, utility bills, and more. (With this program, members may be able to find assistance programs they qualify for, but didn't know about.)

To be eligible for our Silver Rx (PPO SNP) Plan and Gold Rx (PPO SNP) plan, you must reside in the following counties:
SILVER RX AND GOLD RX (PPO SNP) COUNTIES:
Illinois: Brown. Fulton, Morgan, Peoria, Pike, Schuyler, Tazewell, Warren
* The Over the Counter benefits is only available in the Silver Rx (PPO SNP) Plans.
