Medicare Advantage Prescription Drug Plan (MAPD)
Here's a plan for Medicare beneficiaries who have Medicare only
If you have Medicare only, the Care Improvement Plus Medicare Advantage (PPO) Plan was specifically designed for beneficiaries with Medicare only who don't qualify for our Special Needs Plans (such as spouses and caregivers of our Special Needs Plan members). This plan focuses on helping you proactively manage your health, so you can feel your best and stay active.
- If you currently have a Medicare Supplement policy: You may be paying more out-of-pocket for deductibles and coinsurance than with a Care Improvement Plus Medicare Advantage Plan.
- If you currently have a Medicare HMO: You may have more freedom of choice when it comes to the doctors and hospitals you can access with our Care Improvement Plus Medicare Advantage Plan due to its open access provider network for Medicare covered services.
- If you are relying on Original Medicare (Parts A & B) alone: Compared to Original Medicare, with a Care Improvement Plus Medicare Advantage Plan, your level of coverage will increase, while your financial risk for excessive out-of-pocket health care costs will decrease.
Additional services - well beyond what's provided by Original Medicare:
Care Improvement Plus has an Open Access Provider Network, with no referral requirements for Medicare covered services. This lets beneficiaries go to any Medicare-approved provider that will accept payment from our plan. In addition, beneficiaries also get:

- Podiatry care
- 24/7 Nurse Hotline
- Preventive health care
- Emergency care
- Durable medical equipment benefits
- Social Service Coordinators program – to help members better manage out-of-pocket expenses for health care, prescription drugs, utility bills, and more. (With this program, members may be able to find assistance programs they qualify for, but didn't know about.)

To be eligible for our Medicare Advantage (PPO) Plans you must reside in the following counties:
MEDICARE ADVANTAGE (PPO) COUNTIES:
New York: Albany, Erie, Hamilton, Rensselaer, Saratoga, Schenectady
* To be eligible for this plan you must be entitled to Medicare Part A and enrolled in Part B. However, individuals with End Stage Renal Disease are generally not eligible to enroll in the Care Improvement Plus Medicare Advantage (PPO) Plan unless they are members of our organization and have been since their dialysis began.
